Can I use 3 wires on 220v from a genset to the house, or must it be 4 wires?
What is conventional?

What is the conduit? Metal could be considered the ground so the three wires
would be OK. If its not metal then you need to pull a seperate ground. At
least this is what is considered the norm for wiring. Maybe generators are
different? But someone will still need to know what the conduit is.

It won't be conduit. It'll be cord like welder pigtail or dock power cords.

4 wires is the right way to do it.
