You're just restating the 2nd Law of Thermodynamics. The key to this
entropy phenomena is waste energy, which is usually heat and which is
always generated by friction and other undesired reactions. The key to
approaching 100%efficiency is in minimizing friction and "discouraging"
undesired reactions. That's a very difficult thing to do in the
presence of insane temperatures (hence, pollutants are found in high
temperature combustion processes).
