Is there a good method for attaching 1/4" plywood to a concrete slab to make
a smooth subfloor for laminate flooring?

You'd need something more then 1/4". The quarter will bend and follow the
dips/highs of the slab.

If the slab is that rough,look at using a self levelling grout to remove the
peaks and troughs.Or possibly a surface grinder.
In no way however should you mechanically fix a timber floor to a concrete
one--different coefficients of expansion (the wood moves more than the
concrete will during temperature changes).
All laminate flooring suppliers provide an underlay for the floor,this
serves a few purposes--eliminates the variation in level of the base and
sound dampens the floor,and to a certain extent provides insulation between
the cold concrete and the flooring.

Hi Mike,

We did a laminate over a concrete floor and upgraded to the best high
density foam underlayment we could get. It worked fine. Made a nice
"floating floor". If you floor is really bad, try a "sleeper" floor
where you can cut strips of 3/4" plywood and glue them in rows on the
floor and then apply the laminate over. I hope this helps.

This is a thought, what glue will stick to both the wood and concrete
equally well?

Alternatively, you can make a floating subfloor (although I've never
done this myself). You cover the entire floor with two layers of 3/8"
plywood, the top layer turned 90 degrees from the bottom layer. Then
you screw the two layers together with 3/4" screws.

I suggest a PL400 product (tube adhesive) or similar. Local Home Depot
or Lowes would have it.
